in real-time through a first-of-its-kind Patient Portal.
Grand River Hospital (GRH) in Kitchener, Canada, provides patient-centered
care to more than 450,000 residents in the Waterloo Region. Given their
large and expanding patient base, Grand River felt the need for an innovative
application to create a virtual medical ‘facility’ to service
patients 24X7, 365 days a year. The virtual medical ‘facility’
would help the hospital improve the overall patient experience and increase
its capacity to serve the patients better and in real-time.
To create this facility, a senior management team from Grand River, led
by CIO Glen Kearns, attended CTE’s one week Cambridge Executive
Workshop (CEW), an intensive program where corporate executives collaborate
to identify and develop innovative solutions.
Executives and their staff designed the Patient Care Portal –
an Internet based system that enables patients to benefit from the following
features:
- Personalized treatment plan with educational content support.
- Personalized ‘Health Tracker’ - the patient can set custom
parameters that generate a guided calendar for tests and procedures
- Capability to self-monitor side effects of treatment
- Online pharmacy linked tightly to the overall hospital system
- Appointment booking, rescheduling, etc
- Ability to maintain personalized medical information online for sharing
between the patient and hospital medical staff

Patient Care Portal in conjunction with a Management Decision Support
Module is expected to yield an annual impact of 600K with further savings
with increased development through additional revenues and elimination
of operational inefficiencies.
